Request diff between arbitrary revisions

Refactor SHM server to handle both POSIX and SYSV IPC.
Add support for SHM server on Mac OS X.

Work around MacOS X kernel issue - avoid doing multiple calls to ldt
setting kernel API from simultaneous threads.

Merge branch into mainline.

Provide function decl.

Make the address and size of the shmserver allocated address
configurable via the WINEPRELOADER_SHMADDR and WINEPRELOADER_SHMSIZE
environment variables passed to the preloader.
This will simply change the address reserved for the shmserver by the preloader,
and a trickle down effect should follow wherein the server should catch
this address.

This adds support for prelink, execshield and a work around for the legacy va layout problem.
This is done through the implementation for a preloader.

Do not attempt to get the win16 mutex before it's initialized. This
was borking 16 applications.

Finally get around to changing all uses of debugtools.h -> wine/debug.h

before I was sure that WineHQ supported per-application x11drv settings,
since WineHQ has ceased to support x11drv settings (--managed, --desktop)
on the command line, but I suppose the merge can't be postponed forever.
