MMap Fix for MacOS X

I prefer to have "make depend" succeed without having valgrind installed.

Work around Valgrind's lack of support for the sidt instruction.

Fix uninitialized variable warnings from GCC 4.

removed stuff from include/winnt.h that didn't belong there and put it into
port.h include/wine/port.h and include/wine/hardware.h (new).

There's no particularly good reason to be using constructors here. Just
do everything inline.

* change all occurances of *_DEBUG_CHANNEL to WINE_*_DEBUG_CHANNEL
* remove the #define's for *_DEBUG_CHANNEL

Finally get around to changing all uses of debugtools.h -> wine/debug.h

Stripped whitespace from end of lines.

Fake MOVS read/write access to the Interrupt Descriptor Table.
Ought to have more functionality, but this appear to do for what I tested
it on (SIDT -> read int05 descriptor -> cli -> write new int05 descriptor
-> sti -> cli -> write old int05 descriptor back -> sti -> return, that's
all it did, nothing more)
