Add some stub implementations of Win98 functions.

When doing a switch on an unsigned variable, if a case statement is a negative number, GCC just optimizes it away entirely.

* change all occurances of *_DEBUG_CHANNEL to WINE_*_DEBUG_CHANNEL
* remove the #define's for *_DEBUG_CHANNEL

get rid of that stuff completely, it is quite obviously a very broken hack, that is stuffing up other apps. a quick look at msdn confirms this. windows doesn't do anything like it.

only complain about those bad masks if they actually set them.

Finally get around to changing all uses of debugtools.h -> wine/debug.h
